  • Initial Applications: Non-refundable $500.00 registration fee 
  • Renewal Applications: Non-refundable $500.00 registration fee

Required Documents

  • Financial Documents that meet the requirements in Utah Code §13-53-108
  • A copy of the participant disclosure highlighting the required items in Utah Code §13-53-106
  • Proof commercial general liability and umbrella insurance policy providing at least a $1,000,000 per occurrence limit of liability
