How to Submit an Application for Registration Through the Online Application System

All registration applications must be submitted online through the registration portal hereNo paper applications will be accepted.

  • All online applicants must have a UtahID to access our application system. Instructions for creating a UtahID account can be found here.
  • Once applicants have created a UtahID, they can begin the online application process here.


Important:  All documents and information submitted must be in the name of the individual registering as a lawyer referral consultant and not in the name of a corporation, limited liability company, or other entity.

Application Requirements

  • Application Instructions Can be found here
  • Pay an annual registration fee of $200 to the Division of Consumer Protection

  • For initial applications, pay an additional $20 for the background check fee.
  • Submit a fingerprint card from the  Bureau of Criminal Investigation when the initial application is filed.  See fingerprint card instructions here.

  • A surety bond for $50,000.00 and accompanying power of attorney, or a $50,000 cash bond is required at the time of registration and renewal on the Division-approved surety bond form See Division-approved Surety Bond here.  

  • A Notice to be displayed in the Lawyer Referral Consultant’s Office. The division requires a copy in English and in your client’s native language to be displayed. The Notice shall be at least 12x20 inches, Boldface Type, and each character must be at least 1 inch in height and width and include the following:
    • The Lawyer Referral Consultant’s full name, address, and telephone number.
    • The Lawyer Referral Consultant’s agent for service of process
    • The name of the surety bond issuing company and the bond number
    • A statement that the Lawyer Referral Consultant IS NOT an attorney
    • The name of each Lawyer Referral Consultant employed at this location
  • A written disclosure to provide to the Lawyer Referral Consultants Clients. The division requires you to submit a copy in English and your client’s native language. The written disclosure must include the following:
    • The Lawyer Referral Consultant’s full name, address, and telephone number.
    • The Lawyer Referral Consultant’s agent for service of process
    • The name of the surety bond issuing company and the bond number
    • A list of services the Lawyer Referral Consultant provides, and the current total fee for each service.
    • A statement the lawyer referral consultant obtained the client’s signature verifying that the client received the written disclosures before a service was provided. See the application instructions here for a verbatim statement. 

Note: Anything stated in quotation marks on the application instructions form should be stated verbatim on your documents.  

  • A verbatim provision stating the client’s right to rescind. This must be in boldface type on the first page of the contract. See application instructions here for a verbatim statement
  • A verbatim provision stating the lawyer referral consultant is NOT an attorney. This provision must be in no less than 10-point font and boldface type. See application instructions here for a verbatim statement
  • State the purpose the lawyer referral consultant was hired.
  • State the services the lawyer referral consultant will provide and the price for each service.
  • A provision stating, “You may report complaints related to a lawyer referral consultant to the Division of Consumer Protection, by calling toll-free at 1(800) 721-7233 or online at https://dcp.utah.gov.” 
  • A provision that states, “Any complaints concerning the unauthorized practice of law may be reported to the Utah State Bar, by calling (801) 531-9077 or online at www.utahbar.org.” 
  • A signature line for both the client and lawyer referral consultant to sign and date the contract.  

Contents of the written contract shall comply with this section and rules made by the Division in accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act found here.
