Contract Submission

Effective immediately August 29, 2024: the Division will no longer accept any contracts, contract amendments and contract updates via email or mail.  All contracts must be submitted through the online portal.  

If you have not already created a Utah ID, you will need to before you upload contracts. 

To access the online portal you will need to create a Utah ID and then link it to your account. Instructions for creating a UtahID account can be found at:  https://dts.utah.gov/idhelp/. Please select Account Creation. 

Once you have created a UtahID, you will be able to view or amend the contracts by going to your dashboard at: https://utahdoc.mylicenseone.com/ 

1. Once on your dashboard page scroll down the dashboard to your Professional Fund Raiser or Professional Fund Raising Consultant Registration. The page looks like this:

2. Select the three dots on the right side of the registration.

3. Select “Add or View a Contract”

4. Once you have clicked on “Add or View a Contract” it will take you to this page, please click “ADD".

5. Once you have clicked on “ADD”  this will open the contract detail page, please fill it out completely.

6. After filling out the details for the contract you will then upload a copy of the current contract in this box and mark the attestation:

7. Once completed click “FINISH” then you will click “SUBMIT” on the final page.
