Initial Applications: Non-refundable $100.00 registration fee
Renewal Applications: Non-refundable $100.00 registration fee + $25.00 late fee for each month or partial month the renewal is late

Current Certificate of Insurance
Contract that meets the requirements outlined in Utah Code §13-23-3 and §13-23-4
- Health Spas Application Instruction Sheet
A list of each Health Spa service that includes the price and duration
If applicable, a type of surety on the Division's approved form. For additional information please review Utah Code §13-23-5 and §13-23-6.
